Job Title:  Case Manager

Full-time | On-Site

Position Overview

We are seeking a dedicated Case Manager to support youth and young adults in achieving independence, stability, and personal development.

This role focuses on providing strengths-based, trauma-informed services to individuals facing emotional and behavioral challenges.

The ideal candidate will work closely with participants to develop life skills, provide guidance, and connect them with essential services, while fostering a supportive and empowering environment.

Key Responsibilities

  • Conduct assessments and develop individualized service plans

  • Provide counseling, crisis intervention, and conflict resolution support

  • Facilitate group sessions and skill-building activities

  • Assist participants in accessing essential services such as healthcare, education, and employment resources

  • Support youth in obtaining necessary documentation (e.g., identification, social services)

  • Coordinate referrals and collaborate with external service providers

  • Conduct home visits and accompany participants to appointments when required

  • Maintain accurate case records and documentation

  • Participate in team meetings, training sessions, and supervision

Required Qualifications

  • Bachelor’s degree in social work, Psychology, or a related field

Preferred Qualifications

  • Master’s degree in social work, Counseling, Psychology, or related field

Experience Requirements

  • Minimum 1 year of experience in case management or a similar role
