Job Title:  Occupational Therapist (OT) – Home Health | Mileage Reimbursement Provided

Job Type:  Full-Time | On-Site

Job Summary

We are seeking a compassionate and dedicated Occupational Therapist (OT) to provide home health services to patients in their homes.

The ideal candidate will evaluate patients' functional abilities, develop personalized treatment plans, and help individuals improve their independence and quality of life through therapeutic interventions.

Key Responsibilities

  • Assess patients' physicalcognitive, and functional abilities within their home environment

  • Develop and implement individualized treatment plans to improve daily living skills and overall independence

  • Provide therapeutic interventions and exercises to enhance mobility, coordination, and functional performance

  • Educate patients and caregivers on safety practicesadaptive equipment, and techniques that support independence

  • Monitor patient progress and modify treatment plans as needed

  • Maintain accurate and timely

clinical documentation

Experience:

  • Minimum of one (1) year of Occupational Therapy experience (preferred)

Certifications:
