Job Type: Full-Time / Part-Time | On-Site
Position Overview
We are seeking a licensed Physician (MD/DO) to provide primary care services within Skilled Nursing Facility (SNF) settings.
The physician will be responsible for diagnosing, treating, and managing patients with a variety of acute and chronic medical conditions while collaborating with interdisciplinary healthcare teams.
The ideal candidate will have experience in Internal Medicine or Family Medicine and be comfortable supervising Nurse Practitioners (NPs) and Physician Assistants (PAs) .
Key Responsibilities
Clinical Care
-
Evaluate, diagnose, and treat patients in Skilled Nursing Facilities

Develop and implement
individualized treatment plans

Prescribe medications and monitor treatment effectiveness

Conduct comprehensive patient assessments, including medical history reviews and physical examinations

Order, interpret, and evaluate laboratory and diagnostic tests
Patient Management

Monitor patient progress and adjust treatment plans as necessary

Collaborate with nurses, therapists, primary care providers, and other healthcare professionals to ensure
continuity of care
- Provide medical consultation and support for complex patient cases
Documentation & Compliance
- Maintain accurate and timely
medical records

Document patient encounters, diagnoses, medications, and treatment plans

Ensure compliance with healthcare regulations, accreditation standards, and
patient confidentiality requirements
Leadership & Supervision
- Provide clinical supervision and support to Nurse Practitioners and
Physician Assistants
- Promote best practices and
evidence-based patient care
Professional Development
- Maintain medical knowledge through
continuing medical education (CME)
- Participate in professional organizations and educational opportunities

Preferred Qualifications
- 3–5 years of clinical experience in Primary Care, Internal Medicine, Family Medicine, Geriatrics, or
Skilled Nursing Facilities
- Experience working with elderly or
post-acute care populations
- Experience in interdisciplinary healthcare environments
