Job Summary

We are seeking a dedicated Family Nurse Practitioner (FNP) to provide comprehensive primary healthcare services in an outpatient clinical setting.

The ideal candidate will work collaboratively with physicians and healthcare professionals to deliver high-quality patient care, promote preventive health, and manage both acute and chronic medical conditions for patients across all age groups.

Key Responsibilities

• Provide direct patient care, including assessment, diagnosis, treatment, and follow-up

• Perform physical examinations and develop

individualized treatment plans

• Diagnose and manage acute and

chronic health conditions

• Prescribe medications and administer immunizations as permitted by state regulations

• Conduct preventive health screenings, wellness visits, and health education programs

• Collaborate with physicians, specialists, nurses, and other healthcare professionals to coordinate patient care

• Refer patients to specialists or additional services when necessary

• Educate patients on disease prevention, healthy lifestyle choices, and

chronic disease management

• Maintain accurate and timely documentation in Electronic Health Records (EHR)

• Ensure compliance with healthcare regulations, privacy standards, and clinical policies

• Participate in multidisciplinary team meetings and quality improvement initiatives

• Support community health and outreach programs as needed

Required Skills & Qualifications

• Strong knowledge of primary care and

family practice principles

• Ability to assess, diagnose, and manage a wide range of health conditions

• Excellent communication and patient education skills

• Experience working in a collaborative, team-based healthcare environment

• Proficiency with Electronic Health Record (EHR) systems

• Strong organizational, critical thinking, and problem-solving abilities

Education & Experience

• Master's Degree in Nursing (MSN) from an accredited program (required)

• Active Nurse Practitioner license or eligibility for licensure in the applicable state (required)

• National certification as a Family Nurse Practitioner (FNP) (preferred)

Physical Requirements

• Ability to perform routine clinical duties

Work Schedule

• Full-time position

• Primarily Monday through Friday, daytime hours
