Position Overview
We are seeking a compassionate and dedicated Long-Term Care Social Worker to provide psychosocial support, care coordination, counseling, advocacy, and discharge planning services for residents and their families.
This role works closely with interdisciplinary healthcare teams to promote resident well-being, enhance quality of life, and ensure compliance with healthcare regulations and ethical standards.
Key Responsibilities
Resident Assessment & Care Planning

Conduct comprehensive psychosocial assessments upon admission and throughout the resident's stay

Develop, implement, and monitor individualized care plans based on residents' social, emotional, behavioral, and psychosocial needs

Identify barriers to care and coordinate appropriate interventions and support services

Participate in care conferences and
interdisciplinary team meetings
Resident & Family Support

Provide counseling, emotional support, and crisis intervention to residents and family members
-
Educate residents and families regarding available resources, benefits, and long-term care options
-
Facilitate family meetings and assist with conflict resolution when necessary

Promote resident rights, dignity, and
quality of life
Care Coordination & Advocacy

Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated care

Advocate for residents' preferences, needs, and treatment goals

Assist with discharge planning, transitions of care, and referrals to community-based services
Documentation & Compliance

Maintain accurate, timely, and confidential documentation in accordance with organizational policies and regulatory requirements

Ensure compliance with federal, state, and
long-term care regulations
Professional Collaboration

Participate in interdisciplinary care planning and team meetings

Support resident engagement and wellness programs
Required Qualifications
Education
- Bachelor's Degree in Social Work (BSW) (required)
Licensure
Experience
- Previous experience in long-term care, skilled nursing, rehabilitation, hospice, geriatric care, or healthcare social work (preferred)
Skills & Competencies

Strong interpersonal, communication, and counseling skills

Knowledge of long-term care regulations, Medicare, Medicaid, and
community resources

Excellent organizational, documentation, and case management abilities

Ability to work collaboratively within interdisciplinary healthcare teams
Preferred Qualifications

Experience working with geriatric populations

Knowledge of behavioral health, dementia care, and
end-of-life services
- Familiarity with Electronic Medical Records (EMR) systems
