Work Mode:  Hybrid | Home Health Setting

Schedule:  Flexible Hours | Day Shift | Monday through Friday | No Night Shifts | Weekend Availability as Needed

Job Summary

We are seeking a dedicated and compassionate Physical Therapist (PT) to provide high-quality home health care services to patients in their homes.

The ideal candidate will assess, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and enhance overall quality of life.

This position offers flexibility, autonomy, and the opportunity to work closely with patients and interdisciplinary healthcare teams to achieve positive clinical outcomes.

Key Responsibilities

• Evaluate patients' physical conditions and functional limitations

• Develop and implement individualized treatment plans based on patient needs and goals

• Administer therapeutic exercises, manual therapy, and other rehabilitative interventions

• Improve patient mobility, strength, balance, coordination, and pain management

• Monitor patient progress and modify treatment plans as necessary

• Educate patients and caregivers on exercises, treatment plans, and injury prevention

• Maintain accurate and timely clinical documentation

• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care

Qualifications

• Active Physical Therapy License (required)

• Minimum of

1 year of Physical Therapy experience

• Home Health experience

• Strong clinical assessment and treatment planning skills

• Excellent communication and interpersonal abilities

• Ability to work independently and manage a flexible schedule
