We are seeking a compassionate and dedicated Geriatrician (Physician) to provide comprehensive outpatient primary care to adult and elderly patients in a community-based setting in Wisconsin. This role focuses exclusively on patients aged 65 and older, addressing their unique medical, functional, and social needs through patient-centered, high-quality care.

KEY RESPONSIBILITIES :
Provide primary care services focused on geriatric patients (age 65+)
Conduct detailed patient evaluations, including medical history and physical exams
Diagnose medical conditions and develop individualized treatment plans
Order and interpret diagnostic tests, labs, and referrals
Deliver preventive care and counsel patients and families on health management
Coordinate care with specialists and interdisciplinary healthcare teams
Maintain accurate and timely medical documentation
Participate in clinical protocol development and quality improvement initiatives
Provide leadership and guidance to clinical support staff
Supervise physician assistants (PAs) and review clinical documentation as required
Participate in patient education and staff training activities
REQUIRED QUALIFICATIONS
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
Completion of an accredited residency program (Family Medicine, Internal Medicine, or Geriatrics)
Board eligible in Geriatrics – Board certification strongly preferred
Eligibility for medical licensure in Wisconsin
Strong knowledge of geriatric medicine principles and practices
Experience in hospital, outpatient, or ambulatory care settings
Preferred Qualifications
Board certification in Geriatrics
Experience working in community health or with underserved populations
Strong commitment to patient-centered and culturally competent care
Additional Information
