Position Summary
We are seeking a qualified Nurse Practitioner or Physician Assistant to provide high-quality patient care in post-acute and skilled nursing facilities. The provider will manage and coordinate patient care, develop treatment plans, and collaborate with physicians, nursing staff, and facility leadership to improve the quality of life for geriatric patients.
Key Responsibilities
Manage and coordinate patient care in post-acute and skilled nursing facilities.
- Conduct daily patient rounds and develop individualized treatment plans.
- Collaborate with physicians, nursing staff, and facility leadership to ensure coordinated patient care.
- Maintain accurate and efficient clinical documentation using the electronic medical record (EMR) system with dictation support.
Qualifications and Requirements
Licensed Nurse Practitioner (NP) or Physician Assistant (PA).
- DEA certification.
- At least one year of experience preferred.
- Experience in geriatrics, internal medicine, hospital medicine, intensive care, emergency medicine, or primary care is preferred.
- Strong interpersonal and collaboration skills.
