Job Title: Physical Therapist
Full-Time (with Part-Time/PRN options) | On-Site
Job Summary
We are seeking a compassionate and skilled Physical Therapist to provide high-quality care across a range of patient settings, including skilled nursing, assisted living, and memory care.
The role focuses on improving patient mobility, function, and overall quality of life through individualized treatment plans and evidence-based therapy techniques.
Key Responsibilities

Conduct comprehensive patient evaluations, including range of motion, strength, and functional mobility

Develop and implement personalized treatment plans based on patient needs and goals

Apply therapeutic techniques such as exercise therapy, manual therapy, and mobility training

Monitor patient progress and adjust treatment plans to ensure optimal outcomes

Maintain accurate and timely documentation of patient care and progress

Educate patients and caregivers on home exercise programs, injury prevention, and wellness strategies

Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care
Qualifications

Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited institution

Active and valid Physical Therapist license

Experience in geriatric care, skilled nursing, or similar settings is preferred

Strong clinical reasoning and problem-solving skills

Excellent communication and interpersonal abilities

Ability to work independently and as part of a multidisciplinary team
