We are seeking a compassionate and collaborative nurse practitioner or physician assistant to deliver exceptional care in post-acute and skilled nursing facilities. You will build meaningful relationships with patients round the clock and develop tailored treatment plans alongside a dedicated, interdisciplinary healthcare team.
Key Responsibilities:
Manage and coordinate high-quality patient care in a post-acute/skilled nursing setting.
- Conduct daily rounds and develop, implement, and adjust treatment plans.
- Collaborate seamlessly with physicians, nursing staff, and facility leadership.
- Document patient encounters efficiently utilizing an AI-assisted EMR with dictation support.
Qualifications & Skills:

- Required: Active, unrestricted Nurse Practitioner or Physician Assistant license; DEA Certification.
- Preferred: 1+ year of clinical experience. A background in geriatrics, internal medicine, hospital medicine, ICU, emergency medicine, or primary care is a strong plus.
- Technical Skills: Proficiency with EMR systems; comfort with AI-assisted dictation tools.
- Soft Skills: Compassionate, highly collaborative, self-motivated, and excellent communicator.
