Job Title: Occupational Therapist (OT) – Home Health

Employment Type: Full-Time

Work Mode: Field-based (In-home patient care)

Work Schedule: Day Shift (Monday to Friday)

Role Overview

The Occupational Therapist (OT) will provide in-home therapy services to patients, helping them regain independence and improve their ability to perform daily living activities. This role involves assessing patient needs, developing individualized treatment plans, and working closely with patients and caregivers to enhance quality of life.

Key Responsibilities

Patient Assessment & Care Planning

Evaluate patients' functional abilities in a home environment

Develop personalized treatment plans based on patient needs and goals

Continuously assess and update care plans based on progress

Therapeutic Intervention

Implement therapy programs to improve daily living skills (e.g., dressing, bathing, cooking)

Use adaptive techniques and assistive devices to promote independence and safety

Patient & Caregiver Education

Educate patients and caregivers on therapy techniques and home safety practices

Provide guidance on the use of assistive equipment

Collaboration & Documentation

Maintain accurate and timely documentation of patient visits

Communicate effectively with the interdisciplinary healthcare team

Participate in care coordination and case discussions

Qualifications

Education

Graduate of an accredited Occupational Therapy program

Licensure & Certifications

Valid Occupational Therapist license in the relevant state

Current BLS / CPR certification

Experience

Minimum 1 year of experience as an Occupational Therapist

Experience in home health settings (preferred)

Other Requirements

Valid driver's license

Reliable, insured vehicle for travel to patient homes
