Job Title: Registered Nurse (RN) – Patient Care & Clinical Coordination

Job Summary

The Registered Nurse (RN) is responsible for providing high-quality nursing care to residents/patients, ensuring their physical, emotional, and medical needs are met. This role involves direct patient care, care planning, supervision of support staff, and coordination with healthcare teams to promote overall well-being and independence.

Key Responsibilities

Patient Care & Clinical Duties

Provide direct nursing care in accordance with physician orders and established nursing standards

Monitor patient health, identify medical or behavioral changes, and take appropriate action

Assist patients with activities of daily living (ADLs) and promote independence

Administer medications and treatments safely and accurately

Care Planning & Coordination

Develop, implement, and update individualized care plans

Coordinate care with physicians, therapists, and other healthcare professionals

Ensure delivery of preventive, therapeutic, and rehabilitative services

Monitoring & Reporting

Maintain accurate patient records and documentation

Report patient status, changes, and care updates to appropriate stakeholders

Ensure compliance with healthcare regulations and standards

Team Supervision

Supervise and guide nursing assistants and support staff

Delegate tasks effectively while ensuring quality patient care

Qualifications

Required Qualifications

Valid Registered Nurse (RN) license (active and in good standing)

Degree or diploma in Nursing (Associate's or Bachelor's preferred)

Strong understanding of clinical practices and patient care standards

Preferred Qualifications

Prior experience in patient care or senior care settings
